How to use this hospice finder
Hospices tell Medicare which ZIP codes they serve. This tool reads that list, so the hospices shown for your ZIP are the ones that say they will come to a home, assisted living community or nursing facility there. A listing is not a promise, so always confirm by phone that the hospice is taking new patients in your exact location and can start when you need it.
- Type your ZIP code (or the patient’s ZIP, if different) and press Find hospices.
- Sort by any column. Sorting by Visits last days or Care Index is a good start.
- Tick up to three hospices to see them side by side with the Michigan and national averages.
- Call your top two or three and ask the questions below.
What the measures mean
- Hospice Care Index (of 10). Medicare’s summary score built from billing records: nursing visits, visits near death, live discharges, weekend nursing and more. Most hospices score 9 or 10, so a lower score is a reason to ask questions.
- Visits in the last days of life. The share of patients who had a nurse or social worker visit in their last three days. Families tell us this matters most. The national average is 48.3% and the Michigan average is 63.3%.
- Nurse minutes per home-care day and weekend nursing. How much skilled nursing time patients receive on an average routine day, and how much of it falls on Saturdays and Sundays.
- Early live discharges. Patients discharged alive within a week of starting hospice. A high rate can mean admissions that did not fit. Lower is better.
- Family survey stars and recommend rates. From the CAHPS Hospice Survey mailed to family caregivers after a death. Medicare only publishes these once enough surveys come back over two years, so small and newer hospices, including Lily, often show none yet.

Seven questions to ask every hospice you call
- Do you serve this exact address, and are you taking new patients there now?
- How soon can a nurse come for the first visit? Today? This weekend?
- Who answers the phone at 2 a.m.: a nurse, or an answering service?
- How often will the nurse and aide visit in the first two weeks?
- What happens if symptoms get out of control at night? Can you provide continuous care at home or arrange inpatient care?
- Can you work with the patient’s assisted living or nursing facility, and how often do you visit there?
- What support do you offer the family, including respite and bereavement care?

How do I find a hospice that serves my ZIP code?
Type the ZIP into the finder above. It lists every Medicare-certified hospice that includes that ZIP in its Medicare service area, using the August 2026 Care Compare data. Then call to confirm they can start care at your address.
Which hospice in metro Detroit has the best quality scores?
No single hospice leads on every measure. Sort the finder by the measure you care about most, such as visits in the last days of life or the family survey rating, and compare two or three hospices side by side against the Michigan and national averages.
Why do some hospices show dashes instead of scores?
Medicare does not publish a measure when a hospice has too few patients or surveys in the reporting period. Newer and smaller hospices often show dashes for the family survey even when their other measures are published.
Is this hospice comparison tool free and independent?

Does Medicare pay the same no matter which hospice I choose?

Can I switch hospices if I am not happy?
Yes. A Medicare patient can change hospice providers once in each benefit period without losing hospice coverage. The new hospice handles most of the paperwork.
