Lily Hospice Quality Scores on Medicare Care Compare
Published by Lily Hospice from data released by the Centers for Medicare & Medicaid Services. Verify any number at Medicare Care Compare (provider number 231684).
Every Medicare-certified hospice is measured by CMS and the results are public. Most families never see them because they are buried on a government website. We would rather you saw ours. Below are Lily Hospice’s current Care Compare results next to the national and Michigan averages, with a plain-English note on what each one means. All figures are from the CMS provider data files current as of September 2026.
| Measure | Lily Hospice | National avg. | Michigan avg. | What it means |
|---|---|---|---|---|
| Hospice Care Index (0–10) Jan 2023 – Dec 2024 |
9 | 8.8 | 9.0 | A ten-point score Medicare builds from claims data: nursing visits, visits near death, discharges, weekend care and more. Higher is better. |
| Patients visited by a nurse or social worker in the last 3 days of life Jan 2023 – Dec 2024 |
71.3% | 48.3% | 63.3% | The measure families ask about most. It shows whether the hospice was physically present when it mattered. |
| Patients with a visit near death Jan 2023 – Dec 2024 |
89.9% | 89.3% | 91.6% | Part of the Hospice Care Index: the share of patients who received a skilled visit in the final week. |
| Early live discharges Jan 2023 – Dec 2024 |
3.2% | 7.3% | 9.0% | Patients discharged alive within seven days of admission. Lower is better; it suggests careful admissions rather than churn. |
| Treatment preferences documented Oct 2024 – Sep 2025 |
100% | 99.8% | 99.9% | Whether the patient’s wishes about life-sustaining treatment were discussed and recorded at admission. |
| Beliefs and values addressed Oct 2024 – Sep 2025 |
98.6% | 99.3% | 99.5% | Whether spiritual and personal beliefs were asked about, if the patient wanted. |
| Pain screening at admission Oct 2024 – Sep 2025 |
98.6% | 99.0% | 99.2% | Whether pain was screened within the first days of care. |
| Shortness-of-breath screening Oct 2024 – Sep 2025 |
100% | 99.5% | 99.6% | Whether breathing difficulty was screened within the first days of care. |
About the family caregiver survey
Care Compare also reports a star rating from the CAHPS Hospice Survey, which is mailed to family caregivers after a patient’s death. Medicare publishes a hospice’s rating only after a minimum number of surveys have been completed over a two-year window. Lily Hospice is a small, locally owned hospice, and our family survey results are not yet published on Care Compare for that reason. Until they are, the best public record of what families think is our Google reviews, and the best private one is a phone call to a facility or family we have served; ask us and we will connect you.
To see how these numbers compare with every other hospice serving the three counties, use our Metro Detroit Hospice Directory (all 104 hospices, sortable) and read The State of Hospice in Metro Detroit, 2026.
How to read these numbers
No single score tells the whole story. The Hospice Care Index and the last-days-of-life visit rate come from Medicare claims, so they reflect what actually happened, not what was promised. The admission measures (preferences, beliefs, pain and breathing screening) are near 100% for most hospices and mainly confirm that basic assessments were done. A hospice with strong claims-based measures and specific, detailed family reviews is usually a hospice that shows up. If you want to compare us with another hospice, open Care Compare, search their name, and put the pages side by side. We are comfortable being compared.

What we are working on
We publish these numbers because we believe families should choose hospice with their eyes open, and because publishing them keeps us honest. Where a measure is below the state average, we treat it as a to-do list, not a footnote. Our clinical leadership reviews this data every quarter.
