Oakland, Macomb and Wayne counties have 136 Medicare-certified nursing homes: 43 in Oakland, 30 in Macomb and 63 in Wayne. This directory lists each one with its phone, certified beds and Medicare’s star ratings from CMS data as of August 1, 2026. That covers overall, health inspection, staffing and quality stars. How to read Medicare’s star ratings

Medicare gives every certified nursing home a rating from 1 to 5 stars. According to the CMS Five-Star Quality Rating System page, 5 stars means much above average quality and 1 star means much below average. There is one overall rating and three separate ratings behind it:

• Health inspections: what state inspectors found during inspections and complaint investigations.
• Staffing: nurse and aide staffing levels. CMS added weekend staffing and staff turnover to this rating in 2022.
• Quality measures: how residents are doing on a set of clinical measures Medicare tracks.

CMS itself cautions that no rating can capture everything that matters. Specialty care, such as dementia care or rehabilitation, is not in the stars. Neither is how easy the home is for family to visit. CMS notes that a nearby home may be a better choice than a higher-rated one far away, because visits help residents. It suggests visiting in person and talking with local advocates such as the State Long Term Care Ombudsman program.

How hospice works inside a nursing home

A nursing home resident can receive hospice without moving. The facility keeps providing room, meals and daily care. The hospice team adds visits from a nurse, an aide, a social worker and, if wanted, a chaplain. It also provides medications, equipment and supplies related to the terminal illness, and support for the family.

You choose the hospice, not the facility. The hospice and the nursing home need a written agreement to work together. Ask the facility which hospices it works with, and ask the hospice you prefer whether it can serve that home.

Under Medicare Part A, the hospice benefit covers the care team, medications, equipment and supplies related to the terminal illness. Room and board in the facility is not covered by the hospice benefit, so those payment arrangements continue as before. Source: CMS Provider Data Catalog, Nursing homes including rehab services, Provider Information (dataset 4pq5-n9py), data as of August 1, 2026. We included every Michigan nursing home whose CMS county is Oakland, Macomb or Wayne. All 136 are certified for both Medicare and Medicaid. Beds are certified beds, not current residents.

CMS cuts facility names off at 50 characters. Where the missing ending was obvious, such as “Heights” or “Center”, we completed it. Where it was not, the name ends in “…” and the address beside it identifies the home. We corrected two misspelled city names. Nothing else was changed. Corrections are welcome at (248) 955-5100.

Medicare rates each nursing home from 1 to 5 stars. Five stars means much above average quality and 1 star means much below average. There is one overall rating plus separate ratings for health inspections, staffing and quality measures. CMS explains the system here.

How current is this nursing home information?

The ratings, bed counts and phone numbers come from the CMS Provider Data Catalog file with data as of August 1, 2026. Medicare refreshes Care Compare regularly, so a facility’s live Care Compare page may show newer ratings than this table.

Why does a nursing home show no star rating?

Medicare did not publish that rating for the home in this release. Two homes have no overall rating and a few are missing a staffing or quality rating. The home’s Care Compare page gives Medicare’s reason.

Can someone living in a nursing home receive hospice care?
