What we need from you

By phone, we will ask for the patient’s name and date of birth, primary diagnosis and any recent hospitalizations, the attending or certifying physician, insurance, where the patient is now and where they will be receiving care, and the best family contact. If you have a face sheet, H&P or recent notes, we will tell you the secure way to send them when you call. Missing pieces never delay the evaluation; we chase paperwork so you do not have to.

What happens after you refer

  1. Same-day callback. An intake nurse confirms the details and contacts the family.
  2. Evaluation visit, usually within 24 hours, at the bedside or in the home. No obligation for the family.
  3. Admission as soon as the physician order and consents are in place, often the same day. Equipment and medications are arranged before we leave.
  4. Updates to you on evaluation, admission and any change in status, in whatever way you prefer.

Who to refer

Any patient with a life-limiting illness whose physician would not be surprised by a death within the next year is worth a conversation. Common indicators: two or more hospitalizations or ER visits in six months, unintentional weight loss, declining function, difficulty swallowing, uncontrolled pain or symptoms, recurrent infections, or a patient or family who is asking about comfort-focused care. Condition-specific guidance: dementia, heart failure, COPD and lung disease, cancer, and Parkinson’s, ALS and other neurological disease.

How fast can you admit?

Once we have a physician order and the family agrees, we can usually admit the same day, including evenings and weekends. Evaluation visits are typically within 24 hours of your call.

Do you accept patients in facilities?

Yes. We care for residents in assisted living, memory care, adult foster care and skilled nursing facilities across Oakland, Macomb and Wayne counties, and we coordinate with facility staff rather than around them.

What if the patient is not hospice-eligible yet?

Refer anyway. Our Pathways palliative program supports patients who are still pursuing treatment or who do not yet meet hospice criteria, and it makes the eventual transition to hospice much smoother.

What insurance do you accept?

Medicare (Original and Advantage, through the Part A hospice benefit), Medicaid, VA-authorized care, and most commercial plans. If you are unsure, call and we will verify quickly.
