Hospice Care in Canton, MI

Hospice at home when the house is full

A common Canton picture: your mother has a bedroom on the first floor of your colonial off Warren Road, you and your spouse both work, and your teenager helps in the evenings. Hospice from Lily means:

  • A nurse who comes on a set schedule and after hours when needed, managing symptoms and adjusting the plan as your mother changes.
  • An aide for bathing and personal care, so your relationship with your mother stays a daughter’s, not a nurse’s.
  • A social worker who helps you talk to your employer, tap Wayne County programs and plan for what comes next.
  • A chaplain, if your mother wants one, from any tradition or none.
  • A volunteer who can sit with her for an afternoon.
  • Medications for her symptoms, a hospital bed, oxygen and supplies, all delivered.

When hospice is the right call, and how to begin

Hospice is appropriate when a physician certifies six months or less to live if the illness follows its usual course, and your person chooses comfort care instead of curative treatment for that illness. If she is seen at Trinity Health Ann Arbor, Trinity Health Livonia or a University of Michigan Health clinic in the area, her doctor can refer to Lily. Starting is simple: one phone call, a visit from a Lily nurse at your Canton home to explain the benefit and evaluate, and, if you choose, admission the same day. Not sure it is time? The evaluation visit carries no obligation, and we will tell you plainly if hospice is not appropriate yet.

Still in treatment? The Pathways option

Canton families often have a parent in active treatment in Ann Arbor who is miserable from side effects but not ready to stop. Our Pathways Palliative Program sends a clinician to the home to manage those symptoms while treatment continues. It is billed under Medicare Part B and has no prognosis requirement. If hospice later becomes the right choice, the transition is with a team that already knows the family.

Medicare, and what you can expect to pay

Medicare pays Lily directly under the hospice benefit. Covered: the full team described above, medications for pain and symptom control, medical equipment and supplies related to the terminal illness, and bereavement support for the family afterward.

What you might owe: up to $5 per prescription for symptom or pain drugs, and 5% of the Medicare-approved amount for inpatient respite care. Nothing for routine home visits.

The benefit provides two 90-day periods and then unlimited 60-day periods, with physician recertification at each. Hospice can be revoked at any time. Medicaid and most private plans, including Medicare Advantage, mirror the benefit. Source: medicare.gov and the National Alliance for Care at Home.

The break you will need

Caregiving in a busy household is a marathon. Medicare’s inpatient respite benefit places your person in a Medicare-approved facility we arrange for up to five consecutive days per occurrence so you can rest or travel. Continuous home care during a symptom crisis is also part of the benefit. Using these is what keeps home care sustainable.
