Is Lily Hospice a good hospice? An honest answer.

Lily Hospice in Troy, Michigan is a small, family-owned, Medicare-certified and ACHC-accredited hospice with a 4.9-star Google rating from about three dozen families, a Medicare Hospice Care Index of 9 out of 10, and a last-days-of-life visit rate of 71%, well above the national 48%. It is strongest on being present near death and on careful admissions. It is weaker than the national average on nurse minutes per routine day and weekend nursing time, and it has no inpatient unit. Medicare’s family survey star rating is not yet published because Lily is small. Families choosing Lily are choosing a home-based hospice that shows up, not a large system with a building.
Where the numbers say we are strong
Medicare publishes claims-based measures for every hospice. Ours, from the current Care Compare files: patients visited by a nurse or social worker in the last three days of life, 71.3% (national 48.3%, Michigan 63.3%). Hospice Care Index, 9 of 10 (national 8.8). Early live discharges within seven days, 3.2% (national 7.3%), which suggests careful admissions rather than churn. Treatment preferences documented, 100%. Shortness-of-breath screening, 100%. Pain screening, 98.6%.
Where the numbers say we have work to do
We would rather you heard this from us. Nurse care minutes per routine home care day: 9.0, below the national average of about 20. Skilled nursing minutes on weekends: 5.9% of all nursing minutes, versus about 9.6% nationally. Gaps in nursing visits, the share of hospice elections with a stretch of more than a week between skilled nursing visits, 83.2%, which is worse than most hospices. And burdensome transitions type 1, live discharges followed by a hospitalization and readmission to hospice, 16.1% of live discharges, above the national 8.5%.
Context matters. Roughly 70% of the patients we serve live in assisted living, memory care or nursing facilities, where facility staff provide daily hands-on care and our nurse visits are shorter and less frequent by design; the visit-gap and minutes measures count only our own nurses’ time. The live-discharge measures are calculated on 31 discharges over two years, so a handful of cases move the percentage a lot. None of that makes the numbers wrong. Our clinical leadership reviews them every quarter, and weekend nursing coverage and visit cadence in facilities are the two things we are actively changing in 2026.
What families say
Our Google reviews average 4.9 stars. The words that recur are “showed up,” “answered the phone,” “treated him like family,” and “the nurse came that night.” The most-read one is the story behind our first review, a father who walked his daughter down the aisle two days after he could not get out of bed.
What we are not
We have no inpatient hospice house; if a family wants a residential hospice building, Heart to Heart in Troy or Angela Hospice in Livonia have one, and we say so. We are not a hospital-system hospice, and we are not the largest hospice in Oakland County. Medicare’s CAHPS family-survey star rating is not yet published for us because the survey requires a minimum number of completed responses over two years; we expect it to publish as we grow.
Who Lily is a good fit for
A family that wants care at home or in the facility where their parent already lives, a real person on the phone at 2 a.m., an evaluation within a day and admission often the same day, a small team the family gets to know, and a hospice that will show its numbers.
