  • 173 Medicare-certified hospices have a Michigan office, and 52 of them (30%) were certified in 2020 or later.
  • Where the offices are: Oakland 49, Wayne 17, Genesee 15, Saginaw 13, Kent 11, Macomb 7, Kalamazoo 5. More than a quarter of the state’s hospices are based in Oakland County.
  • Family-survey stars: 15 hospices have five stars, 33 have four, 38 have three, 3 have two and 84 have none published.
  • Hospice Care Index: the Michigan average is 9.0 out of 10 (national 8.8). 62 Michigan hospices score a full 10.
  • Visits in the last days of life: statewide, 63.3% of patients had a nurse or social worker visit on at least two of their last three days (national 48.3%). Among individual Michigan hospices the figure runs from 6.5% to 94.3%.
  • Size: the typical (median) Michigan hospice cares for about 49 Medicare patients a day. The largest, Centrica Care Navigators in Kalamazoo, cares for about 588. Together they care for roughly 13,800 Medicare patients on an average day.

Medicare’s star rating comes from the CAHPS Hospice Survey, which is mailed to family caregivers after a death. These hospices hold five stars in the August 2026 data (alphabetical order):

None of the 15 has its main office in Oakland or Macomb County, and one, Angela Hospice, is in Wayne County. That is partly because star ratings need a large number of completed surveys, which favors bigger and older programs. It is one reason to look at more than stars.

Metro Detroit hospices with the strongest last-days visit rates

Of all the published measures, visits in the last days of life varies most between hospices. Among the 73 hospices with a main office in Oakland, Macomb or Wayne County, 52 have a published figure. These ten are highest (Michigan average 63.3%, national 48.3%):

How to read these ratings

  • Hospice Care Index (0 to 10). Ten checks built from Medicare billing records, such as whether nurses visit near death, whether patients are discharged alive unusually early, and whether weekend care happens. Most hospices score 8 to 10, so anything below 8 deserves a question.
  • Visits in the last days of life. The share of patients who got a visit from a registered nurse or medical social worker on at least two of their final three days. This is the measure that varies most from hospice to hospice, and the one families tend to feel most.
  • Family stars and “would recommend.” From the family caregiver survey. Stars summarize eight topics, including communication, timely help and help with pain and symptoms.
  • Patients per day. Size, not quality. Small hospices can be very good; they simply have fewer published numbers.

Four checks any Michigan family can do in ten minutes

  1. Confirm the state license. Michigan licenses hospices through LARA. Search the hospice by name in LARA’s free Verify License tool.
  2. Open its Medicare page. Click the hospice’s name in the table above, or search Medicare Care Compare. Look at visits in the last days of life and the family survey first.
  3. Compare it with two others that serve your address. In metro Detroit, type your ZIP into the hospice finder and tick three.

Medicare's August 2026 data lists 173 Medicare-certified hospices with a Michigan office: 93 for-profit, 38 nonprofit, 2 government-affiliated and 40 listed as other or not yet classified. 52 of them were certified in 2020 or later. 49 have their main office in Oakland County alone.

What is the best hospice in Michigan?

There is no single best hospice, because the best one has to serve your address and answer when you call. A fair shortlist starts with hospices that serve your ZIP code and score well on what Medicare measures: the Hospice Care Index, visits in the last days of life and the family survey. 15 Michigan hospices currently hold five family-survey stars, and they are listed on this page. Then call two or three and ask the same questions of each.

How do I check that a Michigan hospice is licensed?

Michigan licenses hospices through the Department of Licensing and Regulatory Affairs (LARA). Use LARA's free Verify License search (statelicensing.apps.lara.state.mi.us) and look the hospice up by name. Medicare certification is separate; you can confirm it on Medicare Care Compare, which every row in the table above links to.

Why do so many Michigan hospices have no star rating?

Medicare only publishes family-survey stars after a hospice has enough completed surveys over two years. 84 of Michigan's 173 hospices have no stars yet, mostly because they are small or new. No stars means not enough surveys. It does not mean a low score. Lily Hospice, certified in 2021, is one of them.

Are nonprofit hospices in Michigan better than for-profit ones?

On average, Michigan's nonprofit hospices score somewhat higher on Medicare's published measures: visits in the last days of life average 72% for nonprofits and 60% for for-profits, and family-survey stars average 4.1 against 3.5. But the spread inside each group is far wider than the gap between them, so ownership alone will not tell you much. Judge the individual hospice.

All figures come from the Centers for Medicare & Medicaid Services hospice provider data, August 2026 release: Hospice General Information, Hospice Provider Data (Hospice Care Index and visits in the last days of life, 2023 to 2024) and the CAHPS Hospice Survey (October 2023 to September 2025). We included every hospice Medicare lists with a Michigan address and changed nothing except the capitalization of names. Averages by ownership are simple averages of the hospices that have a published value. Michigan and national averages are Medicare’s own.
